How to Remove Eyebags: Transconjunctival (Scarless) vs Transcutaneous (Cutting of skin)
Method 1: Transconjunctival Lower Blepharoplasty (Scarless Eyebags Removal)
This is a minimally invasive procedure performed under local anesthesia with no cutting of the skin and no placement of sutures or stitches. Fats are removed from the pockets located on the inner side of the lower eyelid through a small incision. However, the doctor should have a good understanding of the structures within the eye socket as the procedure is performed near to the eyeball and can be a daunting task for doctors without prior training in eye surgery.
In the hands of an experienced doctor, the outcome of this procedure can be very satisfying with immediate improvement to the appearance and the incision is hidden within the inside of the lower lid, hence the term ‘scarless’.
One of the main limitations of this procedure is the lack of skin removal. As such, individual with excessive skin laxity may not be a good candidate for such a procedure. It is wise to have the eyebags removed at a younger age when the skin elasticity is adequate and negate the need for a more invasive procedure as discussed in the next section.
These are the steps for a scarless eyebag removal:

- Step 1: The position of the eyebags are marked on the surface of the skin

- Step 2: Eyebags are marked and tested to ensure correct marking.

- Step 3: A small incision is made using local anaesthetic to numb the pain. Only the sensation of a needle prick is felt. There is no pain during the incision.

- Step 4: Tiny blood vessels are identified on the surface of the fats. These need to be coagulated in order to prevent any bleeding and to minimize bruising after the operation.

- Step 5: The vessels are carefully coagulated using a special radiofrequency probe to prevent any bleeding.

- Step 6: The eyebag fats are lifted and further vessels are coagulated to ensure a completely bloodless surgery.

- Step 7: The eyebag fats are cut off and removed. These steps are repeated for each eyebag.

- Step 9: This is what the incision looks like after the operation. Stitches are not necessary.

- After the surgery, the eyebags are obviously smaller and no longer bulging.
